Documents required for Basic B.Sc. Nursing:

1. Association CET mark list. Certificate of passing of the qualifying examination and true copy of the statement of marks.

2. MH-CET mark list (If applicable) Certificate of passing of the qualifying examination and true copy of the statement of marks.

3. 12th passing certificate

4. 12th mark list

5. School leaving certificate/ Transfer Certificate, in original, of the last Institute attended.

6. Migration certificate from state board

7. 10th passing certificate

8. Caste certificate (If Applicable)

9. Caste Validity Certificate (If applicable) to support his claim as backward class candidate in specified form

10. Non Creamy Layer certificate (If applicable) Certificate issued by the competent authority stating that the candidates parents do not belong to creamy layer, as per relevant orders of the Government of Maharashtra. (if applicable)

11. Gap affidavit on 100 Rs/- stamp paper

12. Nationality Certificate nationality issued by a competent authority or by a revenue office not below the rank of Tahsildar.

13. Father's/mother's domicile certificate if candidate has not appeared and passed S.S.C Examination and studies two preceding academic years from school located in Maharashtra.

14. Date of Birth Certificate, if birth date is not mentioned in school leaving certificate.

15. Medical Certificate in the prescribed Performa (in original)

Documents required for Non Resident Indians students:

1. Certificate of passing the qualifying examination from an institution out of India.

2. Domicile Certificate in respect of the parent/ Father of the candidate.

3. Certificate from the Indian Diplomatic Mission/ Chancellery or Commission abroad that the father/ Parent of the candidate is of Indian Origin from Maharashtra and at present residing in that country.

4. Copies of the certificates attached to the application form should be attested by the Principal of the School/Institute or a Gazetted officer or special Executive Magistrate.

5. Original Certificates will have to be produced at the time of scrutiny of application prior to admission.

6. Application for admission will not be considered unless it is: Properly and completely filled in. Accompanied by attached copies of the requisite certificate in support of the claims made in application.
