1) A medical fitness certificate issued by a qualified registered medical practitioner.

2) For Std. I : The original Birth Certificate
3) For Std. II to X : The School Leaving Certificate from the last School attended.
4) Progress Report of the last qualifying examination.

If the student has come on a transfer from another state, the School Leaving Certificate must be countersigned by the respective District Educational Officer.
